Liability Waiver Form

In consideration for being allowed to participate in the Activity, on behalf of myself and my next of kin, heirs and representatives, I release from all liability and promise not to sue Vutex and their employees, officers, directors and agents from any and all claims, including claims of Vutex’s negligence, resulting in any physical or psychological injury, illness, damages, death or economic or emotional loss I may suffer because of my participation in this Activity, including travel to, from and during the Activity or while on or upon the premises where the event is being conducted.

I hereby elect to voluntarily participate in said Activity. It is my expressed intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVE, DISCHARGE, and CONVENTION TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be constructed in accordance with the laws of Texas.

I acknowledge that I have been reminded about the importance of drinking responsibly and in accordance with applicable laws. I understand that it is irresponsible and potentially illegal to drink and drive. I am aware that Vutex has negotiated hotel rooms nearby the Activity at a discounted rate to encourage team members to engage in responsible behavior associated with alcohol consumption.

In signing this release, I acknowledge and represent that I HAVE READ THE FORGOING Waiver of Liability and Hold Harmless Agreement, UNDERSTAND IT AND SIGN IT VOLUNTARILY as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing written agreements have been made and I EXECUTE THIS RELEASE FOR FULL, ADEQUATE AND COMPLETE CONSIDERATION FULLY INTENTING TO BE BOUND BY SAME.
